Outpatient Parenteral Anti-Microbial Therapy (OPAT)

OPAT stands for outpatient parenteral anti-microbial therapy, is a form of management of stable infections in the clinic with intravenous/intramuscular injections of antibiotics in an outpatient setting.

It can be delivered in the following settings:
  • By a home/visiting nurse at a patient’s home
  • In the clinic by nurse, doctor or supervised caregiver
  • At an infusion centre (currently not available in Singapore)

  • The different routes of antibiotic administration are:
  • Injections: Intravenous and intramuscular
  • Infusions: Usually over 1-2 hours
  • 24 Hour pumps: freshly prepared by a nurse in clinic/home and changed once daily. Ideally, patients should have a reliable intravenous access like a PICC line or a long central line.

  • Advantages of OPAT:
    1. Cost saving: obviate need for in-hospital stay
    2. Convenience of recuperating in one’s home settings
    3. Allows freedom for patients to do other things: eg. returning to work or run errands
    4. Decrease length of hospital stay also result in decreased risk of catching hospital-acquired infections and iatrogenic risks. (Estimated risk of nosocomial infections is 5-10%)
    5. Happy patient
